Family Chiropractic Care is located at 1020 Forest Avenue in Portland, with convenient free parking next to the building. Dr. John Witkowski established Family Chiropractic Care in 1980. Dr. Witkowski is a licensed Doctor of Chiropractic (Maine License #525), certified levels 1 and 2 by the American Chiropractic Rehabilitation Board, and is a member of the American Chiropractic Association and the Maine Chiropractic Association.

From I-295 Take exit 6B (Route 302) to Forest Avenue - go approximately 3 miles through Woodfords Corner, past Walton Street, and look for 1020 Forest Avenue, a white building on left with a green "Family Chiropractic Care" sign.
