They are used to fix problems related to poor foot biomechanics, the most common of which is a fallen arch.
Problems that can potentially be resolved with a custom orthotic include bunions, Morton's neuroma, metatarsalgia, plantar fasciitis, heel spurs, heel pain, and sometimes even knee, hip, and low back pain.
