DOT drivers are able to pre-fill demographic and health history information here prior to the appointment.
Once an employment/DOT exam has taken place and it has been determined that you are healthy and able to perform the job, you will be given a laminated wallet-sized medical certificate (Form MCSA-5876) that same day.
DOT exam certificates are good for 24 months.
